Now that you’re ready to start your legacy, here are just a few bits of information you should gather first so that your questionnaire process can be completed as quickly and easily as possible.

Full names (first name, middle name or initial, and last name) of people you would like to be your Executor also known as Personal Representative, this person is responsible for making sure your wishes are honored after your death. You will also need to name Successor Representatives as a back-up if your first choice is unable to handle their responsibilities under your will.
Full names of the people you would like to be Guardians and Successor Guardians for your child(ren).
Full names of your beneficiaries, the person(s) who will receive your assets when you are gone.
Full names and addresses for people you would like to be your Property Power of Attorney Agents and Successor Agents.
Full names and addresses for people you would like to be your Health Care Power of Attorney Agents and Successor Agents (and your HIPAA Authorization Personal Representatives)
